New Hire Activation

Use this form to submit relevant information about a new employee hire to the Finance Department, IT Department, and Receptionist. The information in this form will allow these departments to setup appropriate systems and accounts for the user, and to procure equipment.

By submitting this form, the decision to hire is assumed to have been made. At minimum a verbal offer of employment will have been made to the individual. A contract is assumed to be in the works, or is already signed.
