Vaccination Status

In response to the latest requirements established for all post-secondary institutions in Ontario by the Ministry of Colleges and Universities and the Office of the Chief Medical Officer of Health on Tuesday, August 31, 2021, Heritage College & Seminary is required to establish and implement a campus-wide COVID-19 Vaccination Policy.

In accordance with that policy, we would ask that all students, faculty, and staff fill out the following form. Updates on the vaccine policy and COVID-19 related news can be found on the Fall 2021 page.

Agree to Regular Antigen Testing and Reporting(required)
All students, faculty, and staff of post-secondary educational institutions who are not fully vaccinated against COVID-19 or who choose not to disclose their vaccination status must submit themselves to regular rapid antigen testing. You agree to comply with this regulation and submit a rapid antigen test result once every seven (7) days you are on campus using the secure form provided here.
