Antigen Test Report

If you are taking a rapid antigen test provided by Heritage, you are required to report when you took the test and your results. Please note that all information collected is encrypted during transmission and will be kept securely and privately.

If you have trouble accessing the form and need to report results, please contact the Human Resources department at hr@heritagecs.edu.

    In the case that you test positive, please do the following:

    • Go to your home or residence room and self-isolate.

    • Contact Telehealth at 1-866-797-0000 to receive further direction on next steps.

    • If you are a student, please contact the COVID-19 Response Team to inform them of the results and receive guidance at covidresponse@heritagecs.edu.

    • If you are staff or faculty, please reach out to your supervisor to inform them of the results and receive guidance on next steps.
